- Purchase Order Number or Credit Card Information
- Complete Product Number and Quantity
- Physical State of Products (Powder, Chloroform, etc.)
- Most products require dry ice at the following additional cost
- Domestic Shipping charges are between $60.00 and $170.00 (U.S. Dollars)
- First time purchases may be required to pre-pay
- Shipping terms are F.O.B. Origin, Alabaster, Alabama USA.
