12-Ketochenodeoxycholic Acid (12-Oxochenodeoxycholic Acid / 3,7-Dihydroxy-12-Oxocholanoic Acid) is a human metabolite and a derivative of chenodeoxycholic acid (CDCA/chenocholic acid), that bears an oxo-substituent at position 12.
Chenodeoxycholic acid is one of the two primary bile acids (BAs) in the human body, alongside cholic acid. It was first isolated from goose bile, which explains its name (Greek: cheno=goose).
As with all BAs, CDCA is synthesized in the hepatocytes and is a product of cholesterol catabolism. It has numerous effects, typical for all BAs:
- Aiding fats and other sterols digestion and absorption.
- Enhancing bile flow and secretion.
- Participating as a signaling molecule in the regulation of key metabolic reactions that involve cholesterol, glucose, energy, drugs, and other BAs metabolism and homeostasis.
Chenocholic acid is also widely known as a potent detergent that dissolves cholesterol gallstones. In some specific pathological conditions, levels of CDCA raise, and when at high levels, the acid limits the liver synthesis of cholesterol and other toxic BAs. As a result, there is decreased cholesterol concentration of cholesterol in bile and serum. Through this protective mechanism, chenocholic acid manages to prevent gallstone formation.
CDCA has been approved as a therapeutic agent against gallstone formation, as well as other conditions like constipation, bowel dysfunction, impaired colonic peristalsis, etc.
